Ayer Hitam community participations in conservation: a conceptual paper
Introduction: Bullous pemphigoid is a rare acquired, chronic autoimmune blistering disease; which
typically produces tense and large blisters with erosions on skin or mucous membrane. It is
primarily affects elderly patients in the fifth to seventh decade of life and can be fatal,
particularly in patients who are debilitated, with underlying medical problems or left untreated
because of misdiagnosis. There are various factors documented able to facilitate bullous pemphigoid
such as physical agents, viral infections and drug intake including gabapentin. Gabapentin is an
anticonvulsant drugs, but also widely used in the management of neuropathic pain especially in
primary care. Even it is rare, gabapentin has been found to cause bullous pemphigoid from the
previous literatures. Nevertheless, the previous reported cases were mainly involving trunks and
limbs; spared the oral cavity. Report: We herein report a case of a 66-year-old gentleman, who
presented with swollen, itchy and tender upper and lower lips with localized erythematous macular
rashes over left side of his face for two days duration. He had a history of taking oral
gabapentin 300mg three times a day for one week prior to the onset of the lesions. The drug was
prescribed in the primary care clinic for his neuropathic pain secondary to bilateral lower limb
atherosclerotic gangrene. Examination revealed multiple bullous over his swollen upper lips with
desquamative gingivitis. Other system examinations were unremarkable. The offending drug was
immediately ceased and he was then referred to Dermatology team for further evaluation. Result: The
diagnosis of Gabapentin-induced bullous pemphigoid was confirmed by clinical assessment and
histopathological examination. His symptoms gradually improved after a week commencement of high
potency topical corticosteroid. Conclusion: This case illustrates the importance of adequate
physical examination including detailed oral examination to look for the pathognomonic features in
a case suspected to be drug induced rashes. Indeed, adequate knowledge on the side effect of the
possible medication that can induce catastrophe skin problem and prompt action to withhold the
precipitating drug is really essential in primary care in order to save the

Weight management program focusing on body composition and physical activity at a primary care clinic
Overweight and obesity are increasingly prevalent with a ratio of 1 in 2 among adult Malaysian. 65 percent of staff in a primary care clinic was found to be either obese or overweight. A program (Fat 2 Fit) with the aim to empower clinic members to achieve reduction in body fat (BF%) and weight (BMI), improve fitness and build teamwork was initiated in August 2022
Prevalence of physical activity and its association with body mass index among late adolescents in Kuantan, Malaysia
A national data reported more than half of school going adolescents in Malaysia were physically inactive. This study aimed to describe the prevalence of physical activity among late adolescent in Kuantan, Malaysia and to determine factors associated with their physical activity level. A cross sectional study among late adolescents aged 17 - 19 from seven governmental higher learning institutions in Kuantan, Malaysia was conducted from April to October 2018. A validated, selfadministered International Physical Activity Questionnaire (IPAQ) questionnaire was used to measure the level of physical activity among participants. Weight and height of the participants were measured and the body mass index (BMI) was calculated. Chi-square test was used to test for association between physical activity level and BMI of the participants. The association between physical activity and socio-demographic profile was tested using logistic regression analysis. One third of the participants (37.6 %) have high physical activity level while 29.0% of them only practiced low physical activity level. Non-smokers were 0.4 times more physically active than smokers (aOR 0.39, 95% CI 0.19-0.78) while male gender was 2.46 times more active than female (aOR 2.46, 95% CI 1.55-3.92). There was no statistically significant association between physical activity level with BMI of the participants (2 = 0.93, p = 0.63). Only one third of late adolescents in our study practiced high level of physical activity. This is worrying as it may leads to increased number of morbidity and mortality related to low physical activity. Furthermore, physical activity level during adolescence-age group may influenced their physical activity level during adulthood. Therefore, continuous health education focused on the important of physical activity is undoubtedly crucial. Smokers were found to be less physically active than non-smokers. Hence, health education focused on the important of physical activity and the harm of smoking should be targeted simultaneously. Clinica
Covid-19 cluster management in educational institution : challenges in field & digital epidemiology
With students returning to campus in accordance to the national recovery plan, efficient covid-19 management in educational institutions remains paramount. Here we illustrate an outbreak investigation that has been carried out involving students living in hostel setting
Hematidrosis and Hemolacria: a rare case of bloody sweat and tears
Hematidrosis is a very mysterious disorder in which healthy skin secretes blood while hemolacria is bloody tears. Scarce case reports mainly occurred in adolescent Asian girls. The majority were caused by psychogenic
stress although systemic disease, blood problems, and strenuous physical exertion are possible etiologies.
Specific investigation to establish the diagnosis is still a dilemma but a greater challenge in primary care is to
identify and manage the stressor in a teenage patient
DiGeorge syndrome presenting with imperforated anus and recurrent infections in neonatal period: a case report
DiGeorge syndrome or 22q11.2 deletion syndrome (22q11.2 DS) is the most common chromosomal microdeletion disorder. Signs and symptoms are associated with abnormal development of 3rd and 4th pharyngeal pouches. Early diagnosis in primary care through prenatal ultrasound and combined management with multidisciplinary team improve the quality of life of a patient with this rare syndrom
Combating covid-19 : our management at the campus level
In Malaysia, Covid-19 has caused severe morbidity and mortality. Most people have been affected, including the university community. The spread of the virus has put the campus in peril. Covid-19 Kulliyyah (Faculty) Task Force was established to deal with covid-19 issues at our level. After the establishment of the task force, we faced difficulties in managing the students manually, especially in terms of contract tracing
Waiting time among diabetic patients in a local health clinic
Waiting time among diabetic patients is becoming an important issue considering the
increasing number of this population requiring to follow up in primary care clinic.
Diabetic patients have multiple stations to be visited, causing them to have a longer
waiting time compared to other patients. Their dissatisfaction should be investigated
for further improvement, and this should be proved. This study aims to measure the
waiting and consultation time of diabetic patients attending a non-communicable
disease clinic session in Klinik Kesihatan Seremban. A cross-sectional study was done
among 190 diabetic patients in which their waiting and consultation time had been
recorded. SPSS version 23 is used for statistical analysis. Most of the patients were age
64 (6.3%), female (63.7%) and Indian ethnicity (44.7%). 78.4% of them were also
hypertensive, and 50.5% of them had dyslipidaemia. The majority had three
concomitant chronic diseases (46.8%). Maximum waiting time recorded was 104
minutes, with a minimum waiting time of 2 minutes. Average waiting time was
32.73+24.37 minutes. Maximum consultation time was 45 minutes, with an average
time of 9.61+6.27 minutes. There is no significant association between waiting or
consultation time with age, gender, races and number of concomitant illness. This
study showed that the clinic has an acceptable duration of waiting and consultation
time with an average of less than 45 minutes which is fulfilling the requirements set
by the Ministry of Health. The misconception of unacceptable long waiting time
experienced by diabetic patients and chronic disease should be corrected. This study
proved that waiting and consultation time is not determined by the demographics of
the patient and disease only in which patient and public education on the acceptable
waiting time should be implemented regularly
Audit on waiting time in a teaching primary care centre
Waiting time is one of the factors that could affect a patientโs
satisfaction. A study in the Ministry of Health (MOH) primary care clinic showed the average waiting time from registration to consultation was 41 minutes. In IIUM Family Health Clinic (FHC), we adopt MOH client charter for waiting time. The MOH
waiting time for registration, consultation and pharmacy should be less than 15 minutes, 30 minutes and 15 minutes respectively. However, there is no consensus on ideal consultation length. This audit aims to identify patientโs waiting time and consultation length in IIUM FHC in order to formulate our own client charter
